Forms for State Executive and Legislative Candidates

These forms  will help candidates meet their filling and reporting requirements.

 

 

Did you spend over $10,000?
Candidates that expended $10,000 or more the preceding year or expect to expend $10,000 in the current year are required to file campaign finance reports (C3 and C4) electronically. PDC staff provides free electronic filing software and training to all candidates.

 


F-1 Personal Financial Affairs Statement - (MS Word Template), (PDF)
This form is used to report sources of income, real estate transactions, bank accounts, stocks, debts owed, business holdings and business customers.
Due Date: Within 2 weeks of becoming a candidate or being newly appointed. Incumbent elected and appointed officials report annually between January 1 - April 15. Any local elected official whose term of office expires immediately after December 31 shall file a statement of financial affairs for the calendar year which ended on that date.

Identifies relationships with businesses, associations, or other entities and state-level lobbying activity. Incumbent elected and appointed officials also disclose receipt of food, travel, and education payments from anyone outside of their agency. 
Due Date: File with F-1. Any local elected official whose term of office expires immediately after December 31 shall file a statement of financial affairs for the calendar year which ended on that date.


Personal Financial Affairs Statement Short Form - (MS Word Template), (PDF)
A shorter version of the Personal Financial Affairs Statement (F-1) that may be used by annual filers who have filed a full F-1 within the last three years if the information reported on the full F-1 is still accurate or needs only minor changes.
Due Date: Elected officials must file between January 1 and April 15 during the year they hold office. Any local elected official whose term of office expires immediately after December 31 shall file a statement of financial affairs for the calendar year which ended on that date. Appointees to elective office must file within two weeks of being appointed.


C1 Candidate Registration - (MS Word Template

This form contains office sought, reporting option, committee officers, treasurer's name, time and place for public inspection of records.
Due Date: Within 2 weeks of becoming a candidate. For PDC purposes, someone may become a candidate well before a formal declaration is filed with elections officials.

 

C3 Cash Receipts - (MS Word Template), (PDF)
This form is used to report names of contributors, amounts, addresses, occupation, employer.
Due Date: All monetary contributions must be deposited within 5 business days of receipt; fill out a C-3 for each deposit. Prior to June 1, C-3 is due monthly; after June 1, file C-3 reports weekly on Monday.

Details regarding loan payments and loans forgiven and outstanding.
Due Date: File with C3 when loan is deposited.

Attachment Au Auction Report - (MS Word Template), (PDF)
Identifies contributors donating and purchasing auction items. Includes name, address, of the contributor and the market value and purchase price of the auctioned item or service
Due Date: File with C4

 

C4 Summary, Full Report - (MS Word Template), (PDF)
This form is used to report total contributions and expenditures for defined period and overall campaign.
Due Date: Summary of Receipts and Expenditures due with initial C-1 report if contributions were received or expenditures made prior to registration and on the 10th of each month covering the proceeding months activity through June 10th of the election year.

* Candidates not on primary election ballot and general election ballot issue committees continue to file 10th of the month reports.
**  Not required of general election ballot issue committees or continuing political committees or candidates on general election ballot. If a continuing committee is not supporting or opposing candidates or ballot measures this year, it only files C-3 and C-4 reports on the 10th of the month if contributions or expenditures exceeded $200 since the last C-4 report was filed. No 21- or 7- day or post-election reports are required. Candidates participating in special elections file C-4 reports 21 and 7 days before and on the 10th of the first month following the election.

Identifies itemized expenses and deposits made for defined period.
Due Date: File with C4

Schedule B In Kind Contributions, Pledges, Orders, Debts, Obligations - (MS Word Template), (PDF)

Identifies in-kind contributions, pledges, orders placed.
Due Date: File with C4 as needed

Schedule C Corrections - (MS Word Template), (PDF)
Identifies corrections, adjustments.
Due Date: File with C4 as needed

Schedule L Loans Part 2-4 - (MS Word Template), (PDF)
Details regarding loan payments and loans forgiven and outstanding.
Due Date: File with C4 after loan is deposited and continue until loan is paid in full or forgiven.

OTHER FORMS

 

Last Minute Contributions of $1000 or More - (MS Word Template)
Identifies recipients of and contributors giving $1000 or more immediately before primary and general elections
Due Date: Contributors report within 24 hours of or the first working day after giving $1000 or more during 7 days before primary election and 21 days before general election. Recipients report within 48 hours of or the first working day after receiving $1000 or more during 7 days before primary election and 21 days before general election.

 

Special Report E Earmarked Contribution - (PDF)
This report highlights receipt of earmarked contributions given by any contributor with the understanding, intent or instruction that is to benefit another candidate or committee.
Due Date: The report is due within two working days of receiving the earmarked contribution. Also send a copy to the candidate or committee benefited within two days.
